Highland Healthcare Center has an exciting job opportunity for an experienced Social Worker combining the dual responsibilities of Social Services Director and Admissions Director. Qualifications Must meet state and federal requirements for the position and is a licensed Social Worker in the State of Wisconsin. Must have knowledge of state and federal guidelines. Must have demonstrated skills in communication, documentation, assessment and social work methods and techniques. Must be able to relate positively and favorably to residents and families, and to work cooperatively with other associates and personnel of other community agencies. Must be capable of performing the Essential Job Functions of this job, with or without reasonable accommodation. Related administrative experience necessary to perform the job. Social Services Essential Job Functions Include: (1) The responsibility to identify and provide for each residentsí social, emotional and psychological needs, and the continuing development of the residentís full potential during his or her stay at Highland and to assist in the planning for his or her discharge from Highland. (2) The responsibility to develop a social history, social assessment and care plan which identifies pertinent problems and needs, realistic goals to be accomplished and the specific action to be taken in resolution of the problems and or needs upon admission of each new resident. (3) Actively participate in interdisciplinary Care Plan meetings. (4) Document progress notes which relate to each residentsí care plan and when necessary and within policy timeframes. (5) Organize family groups to promote communication, education and support between family members, facility staff and administration, and provide counseling as needed. (6) Assist in the education of the community regarding aging, rights of residents, facility services. Admissions & Marketing Functions Include: (1) The responsibility to direct the facilityís inquiry and admission process and provide customers with facility-related information as the representative. (2) The responsibility to manage and promote the facilities sales and marketing programs designed to achieve monthly census goals for occupancy and a quality mix. (3) The responsibility to train facility personnel regarding effective marketing and admission processes. (4) Provide customers with facility-related information via facility tours, personal visits/assessments, conversations and follow-up.
